The programme is for individuals who have:
- Myocardial infarction
- Percutaneous coronary intervention (PCI)
- Coronary artery bypass grafting (CABG)
- Valve replacement
- Heart failure
- Cardiac transplant
- Pacemaker/ ICD implantation
The programme is also for those who have the following cardiac risk factors:
- Hypertension
- Dyslipidemia
- Diabetes
- Obesity
Inpatient:

Goals
- Promote bronchial hygiene
- Enable safely return to activities of daily living
- Reduce anxiety
- Regain confidence
Components
- Chest physiotherapy
- Mobilisation exercise
- Education
Outpatient:

Goals
- Development of exercise habit
- Improve exercise capacity
- Assist early return to work and leisure activities
- Optimise secondary prevention
Components
- Aerobic, strengthening and stretching exercise
- Education
- Clinical exercise testing for guiding patient’s return to work
Maintenance:

A closely supervised exercise programme, which helps to encourage patients to continue their exercise programme and implement a healthy lifestyle. It is highly beneficial to those who are less motivated to continue exercise on their own.
A referral from your physician is required to participate in this programme.
